The Dream to Live Charitable Foundation provides free daytime care services for children with disabilities in subgroup A at the Inclusive center \u201cWorld of Opportunities\u201d<\/span><\/span><\/a><\/span><\/div>\n

In order for your child to receive this service, you must prepare a package of documents:<\/div>\n
\"\u2705\"<\/span> Passport of one of the parents\/guardians.<\/div>\n
\"\u2705\"<\/span> Tax ID number of one of the parents\/guardians.<\/div>\n
\"\u2705\"<\/span> Child's birth certificate.<\/div>\n
\"\u2705\"<\/span> Documents confirming the child's disability (certificate from the Medical and Social Expert Commission\/Medical and Social Expert Committee).<\/div>\n
\"\u2705\"<\/span> Application for the service (written together with us at the center).<\/div>\n
 <\/div>\n<\/div>\n
\n
\"\u26a0\ufe0f\"<\/span> The service is provided to:<\/div>\n
\u2013 children with disabilities of subgroup A;<\/div>\n
\u2013 aged 3 to 10 years;<\/div>\n
\u2013 residents of Darnytskyi, Dniprovskyi, Obolonskyi, Solomianskyi, and Pecherskyi districts of Kyiv.<\/div>\n
