The Dream to Live Charitable Foundation provides free daytime care services for children with disabilities in subgroup A at the Inclusive center “World of Opportunities”
In order for your child to receive this service, you must prepare a package of documents:
Passport of one of the parents/guardians. Tax ID number of one of the parents/guardians. Child's birth certificate. Documents confirming the child's disability (certificate from the Medical and Social Expert Commission/Medical and Social Expert Committee). Application for the service (written together with us at the center).
The service is provided to:– children with disabilities of subgroup A;
– aged 3 to 10 years;
– residents of Darnytskyi, Dniprovskyi, Obolonskyi, Solomianskyi, and Pecherskyi districts of Kyiv.
We will help you fill out all the documents correctly.
