The form of the letter is free, roughly in two parts:
1 part - the story of your child. Be sure to indicate the child's name, full date of birth, as well as information about the disease: diagnosis, when the diagnosis was made, what was treated, what operations were performed, how the child's well-being changed during this time;

2nd part - justification of the impossibility of paying for the treatment on your own (the letter should contain information about where the parents work and how much they receive).

The conclusion of the letter must include the phrase: "I do not object to the use of information about the child in mass media", number and signature.

3. Супутні документи:

• color photo of the child in good quality (3 pcs);
• a copy of the passports of both parents, if the family is complete, or of one, if the family is incomplete;
• TIN of the legal representative of the child;
• a copy of the child's birth certificate or passport;
• a copy of the child's disability pension insurance certificate (if the child is recognized as disabled);
• a copy of the latest medical report with a diagnosis, on the clinic's letterhead, with the doctor's signature and seal, and an extract from the medical history (the latest on the date of application);
• salary certificate of working family members (salary certificate must be drawn up on the organization's letterhead indicating the position and salary) or the conclusion of local social security authorities about the family's financial situation;
• contact details of the child's legal representative: postal address with zip code and telephone numbers.
• documentary confirmation of the cost of the requested assistance (a bill for medicine or for a rehabilitation course of treatment);
• a copy of the application and denial of assistance from the social security authorities, the Ministry of Health or the region, other funds (if any).

IMPORTANTLY!!! Documents photographed on the phone are not accepted.

The request for help is considered within 10 working days. You will be notified of the results by e-mail, and after a positive decision is made, you will be sent a contract for the provision of charitable assistance for your signature.

To re-apply for charitable assistance to BO BF "Dream of Living" it is necessary:

- the entire list of documents, as for applying for help in the first place.

The Fund reserves the right to refuse assistance if:

• documents are provided partially or their authenticity is doubtful;
• the applicant refuses to communicate with the fund's employees;
• does not actively participate in helping his child;
• does not provide reports on all concurrent meetings;
• does not answer phone calls, e-mails, avoids meetings;
• does not provide the necessary information within three weeks;
